PARENTAL CONSENT FORM

The following form must be filled out by the parents or guardians of every child participating in CFC-sponsored kids' programs for 2011-2012 school year.

We would like every youth or child participating in your programs this year to have one of these forms on file.
This includes those who regularly attend Kids Church, Royal Rangers, Girls Ministries, and Two:9 Youth.

VOLUNTEER WORKER FORMS

Because Christian Faith Center is committed to providing a safe environment for all people involved in our ministries to minors, these forms must be completed by all applicants for any position (volunteer or compensated) involving the supervision or custody of minors, church-sponsored activities, prayer room usage, and other ministries as required by CFC leadership.

The purpose of the following forms is to help the church learn more about its volunteers and to help provide a safe and secure environment for children, youth, and adults who participate in all of our programs and use our facilities. Please provide complete and legible information for all sections.

Note: These forms should be completed prior to the personal interview. The interview takes 10-15 minutes at which time the application will be reviewed. The forms can be completed electronically and sent in via email to Pastor Konrad or brought in at the time of the interview.
